Read the following question and select the correct answer.

Which statement best describes a Stage 2 PU?

Partial thickness loss of skin layers involving epidermis and possibly penetrating into, but not through, the dermis.
Partial thickness skin loss or damage involving epidermis and/or dermis. The ulcer presents clinically as a blister, abrasion, shallow ulcer, without undermining of adjacent tissue. Any of these may have underlying blue/purple/black discolouration or induration.
Epidermis and/or dermis ulcerated with no subcutaneous fat observed.
Partial thickness skin loss involving epidermis and/or dermis. The ulcer is superficial and presents clinically as an abrasion, blister or shallow crater.
