Determine Prevention Management Plan Based on Risk Assessment

Based on the Risk Assessment (RA), a management plan must be developed that eliminates or minimises the risk factors identified in the RA. Different strategies will need to be put in place to counteract the different risks in each situation.

Management strategies generally fall into two major categories:

  • Protect against pressure, friction and shear
  • Maintain skin integrity.

Additional aspects of the plan are the:

  • education of patients and carers so they can assist where possible
  • documentation of plans and the provision of on-going management.
