Responsibility
As with many aspects of health care management, all health care personnel need to understand PU risk factors and be alert to changes, which may alter the patient’s risk levels. 
Due to the complexity of patient/client care, and the type of risk factors involved, it is likely that there will be different team members responsible for undertaking different aspects of the RA.  However, responsibility for RA needs to be allocated within the team to ensure that:

• the full RA is completed
• findings are interpreted
• assessment findings are translated into a management plan.

Timing
The primary team treating a patient/client also needs to determine when the RA will be performed. The Australian Wound Management Association (AWMA) – Clinical Practice Guidelines state that the RA should be conducted:

• on admission
• regularly throughout the length of stay
• following a change in the individual’s condition, especially a deterioration in condition
• prior to, during and following prolonged procedures that involve reduced mobility and hardened surfaces, e.g. surgery and prolonged investigations.
