Wound Assessment

The aim of assessing the wound is to:

Click on each of the headings above for more information about the aim of assessing wounds.

When assessing the wound it is important that you document the following:

  • location
  • stage
  • size
  • wound bed
  • undermining or sinus formation
  • exudate
  • patient's pain (related to the PU)
  • status of surrounding skin.
