What reporting and recording should happen?
In public sector residential aged care, pressure ulcers are reported as part of the Quality Indicators program.

It is very important that the staff you are working with are aware if a resident is in danger of developing a pressure area. While handover is the obvious time to inform everyone, it is also important to let other staff know as soon as a resident at risk is identified. This allows them to monitor the resident and undertake additional pressure area preventative action as needed. Recording information on the resident’s information materials is another way of ensuring preventative action is taken.
It is important that both clinical and non-clinical staff are aware if a resident is in danger of developing a pressure area and of the preventative actions that are being taken.
Talk to your team leader to ensure you know the answers to the following questions:
- Find out how your service records and reports information required as part of the Quality Indicators.
- What are the reporting and recording requirements for you where you work?
- Who do you report to?
- What do you include in your report?
- How do you report?
